Scholarship Terms Acknowledgment*

By accepting scholarship funds, the recipient agrees to attend the Chapter's Annual Dinner Meeting for recognition (unless otherwise approved), and submit a brief written testimonial within three months describing how the scholarship supported their development. Failure to meet these terms may result in returning the scholarship funds.
